Requesting a House Resolution

 

 

 

My office provides House Resolutions for special events and occasions.  Examples would be things such as:

 

Ø      50th Wedding Anniversaries (or more)

Ø      Birthdays 70 years of age or more

Ø      Retirements

Ø      Special occasions and accomplishments

Ø      Eagle Scout Awards

Ø      Heroic Actions

 

I have also awarded them for state-wide academic and sporting accomplishments, to members of the top-ten in graduating classes, just to name a few examples.

 

If you would like to request one of these beautiful documents for a friend or family member, or even for yourself, at the bottom of this page are the forms that you can download and fill out.  You can also contact me at (573) 751-1446 or by e-mail at david.day@house.mo.gov and let me know you are interested in a House Resolution and if e-mailing, a way to contact you.  We will then get the proper form to you so you can provide us the required information.

 

Please remember when requesting a House Resolution that we do reserve these for very special occasions and the recipient must live in the 148th District.  These Resolutions are very beautiful, official documents that are signed by myself and the Speaker of the House.

 

When requested to do so, I try to present the Resolutions in person when scheduling can be worked out.

 

 

 

 

  

FORMS TO REQUEST A HOUSE RESOLUTION

 

You will need Micrsoft Word to open the files below.  Click on the correct form, fill out as much of the

form as possible and either save it and send to angie.thessen@house.mo.gov along with

your contact information or print and mail with your contact information to:

Angie Thessen at (573) 751-1446 and she will be happy to assist you.

It normally takes 7 - 10 days to produce a House Resolution from the time

the information is received and approved.

 

Birthday:  For birthdays 70 years and older.

 

Eagle Scout:  For young men achieving the rank of Eagle Scout

 

Girl Scout Gold:  For young ladies achieving Girl Scout Gold

 

Wedding Anniversaries:  For anniversaries of 50 years or more

 

Retirement:  For individuals retireing

 

Sports:  For outstanding performance in sports

 

Generic:  This form is for any event that does not fit in the above catagories
